A Certified Supportive Living Facility
Oak Wood Estates Retirement Village is a certified Supportive Living facility. It opened in November 2009, next to Oak Lane Nursing & Rehab in Stronghurst, IL. Since 1989, Henderson County Retirement Center has been serving the needs of Henderson County & surrounding counties in Illinois. Oak Wood Estates offers spacious suites with views of our beautiful grounds. We offer many amenities; just a few are home cooked buffet meals, cable TV, wireless Internet, laundry services and many more. Pets less than 20 pounds are also welcomed as long as the residents are able to care for them. We have 24-hour care with a pleasant and friendly staff in a homelike environment.
Oak Wood Estates Retirement Village is a participant in the Illinois Department of Healthcare and Family Services Supportive Living program. This program is designed as an alternative to skilled nursing. It brings affordable housing and a wide array of services to seniors 65 and older.
Oak Wood Estates welcomes low income Senior Citizens with minimal physical disabilities. The main advantage of being a Supportive Living facility is that the Illinois Medicaid program will pay for a portion or all of the medical care for eligible residents. Oak Wood Estates accepts Private Pay, Illinois Medicaid, Long Term Care Insurance and Veterans Benefits.
Oak Wood Estates caters to individuals who require a little extra help but insist on staying independent in a maintenance-free environment. At Oak Wood Estates, you will be surrounded by a caring and compassionate staff with a vast array of services and amenities in a 20-bed facility.
Oak Wood Estates is certified by the Illinois Department of Healthcare and Family Services and is affiliated with Life Services Network and Affordable Assisted Living Coalition.
We are a not-for-profit facility, which is governed by a nine member Board of Directors from the surrounding communities. Meetings are held annually to elect directors and to be brought up-to-date on the undertakings of the facility.

Oak Wood Level of Care
Three Levels of Care
Oak Wood Estates provides 3 levels of care based on the individual needs of each resident. The daily rate is based on the level of care needed. Level 1 is the base level and very limited assistance. Level 2 is moderately increased assistance on reminders to perform activities of daily living. Level 3 requires frequent assistance and support with activities of daily living including cueing or increased assistance with health needs.
Oak Wood Estates Level 1 (Base Rate) is provided to all residents.

(Residents are responsible for telephone service if they elect this service.)
Level One
Minimal Care
Occasional supervision, assistance or reminders to perform some daily activities but is independent in other activities. Will be charged for ancillaries.
Level Two
Moderate Care
Requires more substantial support with some activities of daily living, while needing only minimal assistance with others. Will be charged for ancillaries.
Level Three
Heavy Care
Needs frequent and comprehensive assistance with activities of daily living. Staff will assist with dressing, assist with incontinence care and one person assisted transfers, example, from bed to wheelchair. Will be charged ancillaries.
Note: Level of Care will be determined from an assessment scoring tool. If a resident requires more than level three care, the provider will do a significant change assessment. Will meet with resident and family to discuss if Supportive Living is appropriate for the resident.
